Domicile certificate issued by the local RW or RT of the location where the company will be located.

Scan of Family Certificate (Kartu Keluarga) of the person in charge for company establishment

ID of Shareholders

If the shareholder is Indonesian:

Scan of Shareholder's National ID (KTP)
Valid Tax Registration Number (NPWP)

If the shareholder is foreigner:

Scan of Shareholder's passport identity page
Copy of Article of Association and Director Listing
