Family Certificate of Person in Charge
Scan of Family Certificate (Kartu Keluarga) of the person in charge for company establishment
ID of Shareholders
If the shareholder is Indonesian:
- Scan of Shareholder's National ID (KTP)
- Valid Tax Registration Number (NPWP)

If the shareholder is foreigner:
- Scan of Shareholder's passport identity page
- Copy of Article of Association and Director Listing
